| Description | Medical family health team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness * team includes six family doctors, 2 nurse practitioners, three registered nurses, one RPN, a mental health counselor and a pharmacist * memory clinic fall flu immunization clinics * call for hours Services include: Acute Care - Care for any new injury or illness, any urgent change in a chronic condition, and any failure in palliative care Chronic Disease Prevention and Management Program - Sessions to learn ongoing self-care * includes information about coping, health guidelines, medication usage, working with health professionals, and informed treatment decisions Normal Development, Prevention and Screening - Care of healthy people, preservation of good health, detection of conditions which may not be apparent yet Palliative Care - Management of symptoms where the disease causing the symptoms cannot be cured Services are provided at the office, at home or hospice (palliative care clients), at the Oakville Trafalgar Memorial Hospital (OTMH), at retirement homes, and at Post Inn Village Long Term Care * family practice services include acute episodic care, chronic disease management, normal development prevention and screening, and palliative care * long term care patients, admitted patients and palliative clients (at home or hospice) have access to their family physicians through Dorval Medical 24/7 |
