Description Referral program to provide individuals with Mild Cognitive Impairments (MCI), Alzheimer's disease and related dementias, and their caregivers with education and support as early as possible in the disease process and throughout the continuum of the disease * individuals and families are connected with support from a Counsellor when a First Link Referral is made by a health care professional or community agency * Counsellors offer individuals and families information regarding the disease, service/resources that are available, educational opportunities and link with an Outreach Counsellors for long-term support.
Hours Mon-Fri 8am - 4pm
Areas Served Etobicoke South ; Peel Region ; Behavioural Supports Ontario First Link Referrals (only BSO referrals) - additionally serve Oakville, Milton, and Georgetown
Eligibility Individuals diagnosed with a Mild Cognitive Impairment, Alzheimer Disease or related dementia and their care partners.
Application Referral required.
Fees None - First Link Program, Counselling Services or Education
Languages English ; French ; Multiple languages available upon request.
