Description Referral program that will link individuals and families affected by Alzheimer's disease or related dementia to a community of learning, services and support 
provides access for individuals with dementia and their caregivers to comprehensive and coordinated services 
people and families are connected with support from a counselor when a First Link Referral is made by a health care professional or community agency 
caregiver support groups and counseling sessions 
increased effective and efficient utilization of community resources 
 
One-on-one counseling and group supports for caregivers, including family and friends
Hours Mon-Fri 8am - 4pm
Areas Served Halton Region ; Peel Region ; Behavioural Supports Ontario First Link Referrals (only BSO referrals) - additionally serve Oakville, Milton, and Georgetown
Eligibility Individuals diagnosed with a Mild Cognitive Impairment, Alzheimer Disease or related dementia and their care partners.
Application Call * referral not required
Fees None - First Link Program, Counselling Services or Education
Languages English ; Multiple languages available upon request.
