Description Comprehensive assessments by a nurse practitioner or geriatrician, and a physiotherapist, with recommendations for treatment  
 
Strong and Steady Program - Two-hour education and exercise session, twice a week for six weeks, with a follow-up visit three months after the program is completed * participants will improve strength and balance, learn how to make their home safer, find out their risk of having a fall, and learn many ways to protect themselves  
 
 
Assessments take place at the Queensway Health Centre site or the Credit Valley Hospital site 
 
Exercise and education classes take place at either one of these two locations: 
Carmen Corbasson Community Centre, 1399 Cawthra Rd, Mississauga, L5J 4L1 
Meadowvale Community Centre, 6655 Glen Erin Dr, Mississauga, L5N 3L4
Hours Urgent Care Centre: 8am-10pm
Areas Served Mississauga ; Toronto (Etobicoke West Mall)
Eligibility Ages: 65 year(s) and up 
 
Medically stable adults 65 years and older, who live in the community and want to stay strong, steady and independent * must be well enough to walk 25 meters (with a walking aid if needed) and learn new information
Application Medical referral required
Fees None - most services covered by OHIP
Languages English ; Interpretive services available
