Description | Program for seniors experiencing serious and persistent mental health concerns which include: depression, mood changes, dementia, responsive behaviours, and/or anxiety Seniors having challenges accessing supports due to geography, isolation or changes in informal support system are considered a priority for this service •makes referrals to, and collaborates with, community partners such as Home and Community Care Support Services Central West, Seniors Day Programs, Alzheimer’s Society of Dufferin and local hospitals •education and support to the client and care team, to ensure clients have the necessary services and supports to live successfully in the community and to maintain/improve quality of life |
