Description Provides community-based assistance and advocacy around accessing and maintaining housing, community resources and social services 
Service include: 
•individuals and family assistance with addiction, mental health and other concerns related to homelessness 
•access to nursing care through wound care and foot care clinics 
•street-based Outreach assistance to engage individuals/families towards obtaining and/or stabilizing housing 
•"No wrong door” approach for individuals/families seeking services 
The multidisciplinary team is a partnership of the Region of Peel, Peel Addiction Assessment and Referral Centre(PAARC), Services and Housing In the Province (SHIP) and CMHA/Peel Dufferin
Hours Administration: Mon- Fri 8:30am-4:30pm * Sat, Sun 11am-3pm 
Mobile Outreach: Mon, Wed, Fri 6pm-9pm * Tue, Thu 1pm-4pm
