Application | Complete referral form and fax (905-849-0424) to Halton-Peel Community Aphasia Program for review and determination of eligibility to the program. Referrals can be made by Professionals in the community (Discharge Planners, Speech Language Pathologists, Social Workers, Physicians, etc.), Self-referrals (clients can refer themselves to the program), Family or Friends of the person with aphasia |
