Description | Two ACTT teams serve North Peel and North Mississauga * address the ongoing complex needs of people with a mental illness living in the community * offer community-based treatment, rehabilitation and support. Multi-disciplinary teams comprised of psychiatrists, psychiatric nurses, occupational therapists, social workers, peer support workers, addiction/concurrent disorder counselors, vocational counselors, mental health clinicians, clinical leadership Offer assistance with: •daily living •employment and education •budgeting •integrated treatment for co-occurring substance use •disease prevention, supportive counseling and health training •medication monitoring and management Supports available 24 hours a day, 7 days a week |
