| Description | Support for clients with the initial needs assessment and connection with appropriate resources * at the end of the visit clients will have a support plan in place Steps involve: •initial assessment •service planning •service partner referrals •support plan in place Resource connection: •wellness and support groups •counselling and psychotherapy services and community referrals •support for people who have experienced abuse |
