|Description||Osler offers a variety of mental health services for adults on an outpatient basis at both Brampton Civic Hospital and Etobicoke General Hospital. Counselling is provided on a group basis wherever possible. Short-term individual counselling is also offered.
Referring psychiatrists or family physicians should complete a Centralized Intake and Triage Referral Form and fax it to either:
Etobicoke General: 416-747-3350
Brampton Civic: 905-494-6715
Referrals are welcome for patients and physicians within Osler’s catchment area, that is, the Central-West Local Health Integration Network (CW-LHIN). The CIT process Clinicians screen the patient and provide a brief assessment and referral.
