Description Counselling and support for people with type 1 diabetes and type 2 diabetes (on insulin or insulin pump), gestational diabetes * help to patients and families to understand diabetes and how to self manage the disease - test blood sugars, take insulin, manage medications, make healthy food choices 
 
A diabetes nurse and dietitian will assess patient's health and provide education tailored to your needs. Other services include: 
appointments conducted in person 
insulin pump program 
insulin adjustment classes 
gestational diabetes classes 
paediatric diabetes program 
transitional program for young adults moving from a paediatric program 
referral to a social worker 
in-services provided to schools and daycares (in-person or virtually) 
in-services provided to health care providers (hospital or community)

Hours Mon-Fri 8am-4pm
Areas Served Brampton (City of) ; Caledon (Town of) ; Etobicoke - and area ; Malton ; Woodbridge
Eligibility Adults at risk of diabetes, pre-diabetes, type 1 diabetes, type 2 diabetes and gestational diabetes
