Description Services to individuals, their caregivers, community groups and employers * help to manage diabetes and prevent rapid deterioration that affects quality of life * assist people diagnosed with pre-diabetes to delay development of disease  
Program and Services include:  
•diabetes support group  
•one-on-one counselling sessions with Registered Nurse and Registered Dietitian 
•Diabetes drop-in 
•meter trainings 
•foot care  
•insulin starts 
•exercise programs 
•grocery store tours  
•nutrition and heart health presentation 
Individuals and area serve: People diagnosed with Type 2 diabetes and pre-diabetes living in the north west corner of the GTA. Refugees and newcomers are welcome. Doctor’s referrals and self-referrals accepted. Health insurance not required  
Diabetes Education Centre 3 locations:•Rexdale CHC, 8 Taber Rd, Etobicoke 
•Rexdale Community Hub, 21 Panorama Court , Office 25, Etobicoke 
•Rexdale CHC Satellite, 2267 Islington Ave W, Unit 14
Hours Mon, Tue, Thu, Fri: 9am-5pm; Wed 9am-8pm
