| Description | Clinic for youth who: have been diagnosed with a mental health disorder and are at high risk of harming themselves or others, or are at risk of developing impairments due to a mental health diagnosis TAY patients receive helps to: be involved in their own care learn new skills and coping strategies maintain healthy lifestyles return to school, work and/or daily life smoothly transition into adulthood The clinic offers: virtual care via telephone or video call platforms (e.g. OTN, Zoom) medication education and management psychosocial assessment up to ten individual counselling sessions and/or group programming. Family members may also be invited to attend individual sessions support for transitions to adult mental health care, addictions services, educational and/or employment opportunities referral to community services flexible hours, including evening and weekend appointments Services are offered for up to six months |
