Description Clinic for youth who: 
 
have been diagnosed with a mental health disorder and are at high risk of harming themselves or others, or 
are at risk of developing impairments due to a mental health diagnosis 
 
 
Helps patients to: 
 
be involved in their own care 
learn new skills and coping strategies 
maintain healthy lifestyles 
return to school, work and/or daily life 
smoothly transition into adulthood 
 
 
The clinic offers: 
 
virtual care via telephone or video call platforms (e.g. OTN, Zoom) 
medication education and management 
psychosocial assessment 
up to ten individual counselling sessions and/or group programming. Family members may also be invited to attend individual sessions 
support for transitions to adult mental health care, addictions services, educational and/or employment opportunities 
referral to community services 
flexible hours, including evening and weekend appointments 
 
 
Services are offered for up to six months
Hours Mon-Fri 8am-4pm
Areas Served Peel Region
Eligibility Youth aged 16-24 years * live in the Central West Region * have one of the following: a major mental illness ; new and emerging mental health concerns ; suicidal threats and or ideation ; concurrent substance use
Application Youth can request a referral to this program from their family physician or psychiatrist
Fees Most services covered by OHIP
