* space for art and music programs
Hours Administrative: Mon-Fri 8:30am-4:30pm

Eligibility Older adults experiencing mental health issues
Application Applications managed by Services and Housing In the Province (SHIP) 
Referrals accepted from family physicians, hospitals, partner agencies, family members and significant others or applicants 
Requirements: Signed Consent to Disclose Personal Information Form (Original, not copy or fax) has to be submitted before application can be processed 
Complete SHIP application form 
A psychiatric report to be submitted to Central Intake within six months of the date of application submission
