| Description | Diagnostic imaging clinic * outpatient services angiography bone density CT scan fluoroscopic procedures general radiography (x-ray) mammography (including Ontario Breast Screening Program) high risk breast screening MRI nuclear medicine ultrasound NOTE: When you arrive for your appointment, please register at a self-serve kiosk located in the hospital's main entrance Requisition forms or physician and referral information for Diagnostic Imaging can be found in the Find a Doctor of Service directory Locations: For Nuclear Medicine and Mammography: Physician written referral required * enter through main entrance facing Bovaird Dr; take the Leaf Elevators to the 2nd Floor, turn right as you exit the elevator, then turn left and follow the signs For all other tests: Physicians written referral required * enter through main entrance facing Bovaird Dr; turn left at the elevators and follow the signs, the Diagnostic Imaging Department is on the 1st level in the main atrium |
