Description | Day, evening and night respite for people caring for the elderly or discharged adults who require special care * personal support workers assist with activities of daily living Service operational every day 24 hours a day, made up of personal support workers, respite service providers, a case manager specializing in mental health and a respite services coordinator * on-call system in place for emergencies Personal Support Workers (PSWs) work in the three-bed, two-bedroom facility to provide personalized attention and intensive support with all activities of daily living, including: baths clothing medication reminders nutritious snacks and meals tailored to each client's dietary needs and restrictions case management and referrals Program Criteria: be 55 years of age or older and need care need special care but not admission to hospital client discharged from hospital and not quite ready to be on their own in their own home |
