Description	The program offers the services of two professionally designated Psycho-social Spiritual Care Coordinators Services include: •Compassionate, non-judgmental, empathic listening presence •Support for the emotional and spiritual needs of individuals and families to explore end of life issues and feelings to find inner peace and healing •Coordination of the spiritual and religious needs and practices of individuals and families •Connection with religious leaders and faith communities at the requests of an individual •Assistance to residents and family in planning after life rituals, memorials, and funerals •Bereavement counselling and grief support groups Virtual and In Person services available, as well as 1:1 and groups
Hours	Administration: Mon-Fri 9am-4:30pm
Areas Served	Brampton ; Caledon ; Dufferin County ; Etobicoke North ; Malton ; Woodbridge
Eligibility	Anyone who has suffered through grief and loss (bereavement) or spiritual distress
