Description Community based care and residential care to people and their families touched by life-limiting illness  
Residential care is provided by a team of palliative care professionals when life expectancy is three months or less

Hours Administration: Mon-Fri 9am-4:30pm 
Residential Program: Daily 24 hours

Eligibility People living with the challenges of life-limiting illness and their families * anyone who has suffered through grief and loss (bereavement) or spiritual distress
Application Community services: Referrals will be accepted from a person requiring service, physicians, family members, friends, spiritual and religious leaders and health care professionals * people being referred to the program must have approval from the Power of Attorney and accept the services based on the established criteria * admission is based upon medical, physical, emotional and spiritual need
