Description & Services
| Description | Community based care and hospice residence care to people and their families touched by life-limiting illness. Residential care is provided by a team of palliative care professionals when life expectancy is three months or less. Services and Programs: • Bereavement Care • Complementary Therapies • Grief Support for Children and Teens • In-Home Volunteer Visiting • Psycho-Social Spiritual Care • Residential Care Program • Social Work and Care Coordination |

| Eligibility | People living with the challenges of life-limiting illness and their families * anyone who has suffered through grief and loss (bereavement) or spiritual distress. |
