| Description | The Supportive Care Team is composed of professionals who specialize in providing emotional support, education, and compassionate care for people living with a serious illness, their caregivers, and those grieving a loss Support provided to: •anyone living with a serious illness •caregivers and family members •anyone grieving a loss Caregiver Support •education and guidance to help caregivers feel prepared •emotional support and counselling •connections to community resources •caregiver support groups Grief Support - provide a safe and caring space for people of all ages to: •share their feelings and experiences •learn about grief and ways to cope •connect with others who understand •receive individual or *group support HUUG - Help Us Understand Grief The HUUG Program supports children, youth, and caregivers when someone they love is seriously ill or has died Serious Illness Support - for individuals and families in the community coping with serious illness, caregiving, and loss •emotional support and counselling through difficult times and life changes •help understanding options and making decisions •support navigating healthcare systems and community resources •education about illness, end-of-life planning, and grief •support with advance care planning and legacy work |
