| Description | Social Work supports and services include: •One-to-One Supportive Counselling Topics may include: •living with acute and chronic health challenges; coping with illness-related anxiety and depression; adjusting during times of transition; dealing with anxiety related to testing and procedures, and with fear of dying; facilitating legacy work projects •managing caregiver responsibilities and avoiding burnout; dealing with anticipatory grief •coping with bereavement, loss and grief Additional Social Work services and supports include: •Support Groups •Assistance navigating the health care system •Advanced Care Planning •Patient Advocacy •Guidance on funding sources and assistance with the application process •Linkages and referrals to complementary supports •Education and Resources |
