Description This community program starts when coordinator is assigned to a client  
After a home visit and complete assessment, coordinator fully explains community program options such as: 
•Spiritual Care Services  
•Bereavement Support  
•In-home Visiting Support Program  
•Complementary Therapies  
The social worker/coordinator will provide: 
•one-on-one supportive counselling 
•make referrals as necessary to other available services within the community 
•scheduling of Visiting Volunteers and Complementary Therapies
Hours Mon-Fri 9am-4:30pm
Areas Served Bolton ; Brampton ; Caledon ; Dufferin County ; Etobicoke North ; Malton ; Woodbridge ; Community Program: Residents of Caledon and Brampton ; Residents of the Central West LHIN  
Residential Program: Anyone eligible for LHIN services with a valid Ontario Health Card.
Eligibility People living with the challenges of life-limiting illness and their families * anyone who has suffered through grief and loss (bereavement) or spiritual distress
