Description & Services

Description Provides education to enhance the knowledge and skills for staff working in primary care clinics, community support agencies, who service older adults with cognitive impairment due to mental health, dementia, addictions or other neurological conditions 
 
These service can be used by: 
•Primary care clinics including Family Health Teams and Community Health Centres 
•Seniors' wellness programs 
•Seniors' assisted living programs 
•Community care agencies 
•Assigned adult day programs 
•Family Caregivers
Hours Mon-Fri 8:30am-4:30pm
Areas Served Etobicoke North
Eligibility Must be a government funded agency such as: Home and Community Care Support Services, Adult Day programs; Homemaking agencies; Supportive Housing; Respite Care programs dealing with dementia and mental health issues
Application Referrals can be made by anyone * within a government funded community agency * who has identified a learning need related to the mental health of the elderly * or anyone concerned about an older adult who may have a psychological, behavioural or memory problem
