| Description | Provides: •Education to enhance knowledge and skills of staff •Consultation about learning needs •Care plans and programs related to challenging behaviours of clients / residents •Assistance with referrals to services and resources Psychogeriatric Resource Consultant to Long Term Care and Community Support Services and Health Partners This program is cooperation between Punjabi Community Health Services and Indus Community Services. |
