Description Home and Community Care Support Services Central West nursing clinic  
Integrated Care Clinics provide a variety of specialized nursing services to patients of all ages. Depending upon the specific clinic, these services may include: 
•Wound care 
•Drain care 
•Central line maintenance 
•IV therapy and IV access maintenance 
•Chemotherapy disconnects 
•Lymphedema care 
•Diabetes management counselling and education 
•Education on self-management including but not limited to diabetes and injection 
Clinic is staffed by qualified nursing team, primary care access is not available
Hours Daily 8am-9pm (includes holidays) * Daily last appointment scheduled for 8:30pm * Holidays Hours: 8am-9pm

Eligibility •not open to general public •must be assessed and referred to the clinic by Home and Community Care Support Services Central West staff •visit to clinic by appointment only, no walk-ins accepted •patients must be covered by OHIP
