Description Service for people who are homeless, at risk for homelessness and under housed  
 
To ensure easier access to the homeless/at risk of homelessness/ under housed population, the agency provide health assessment and referrals at the Regeneration Breakfast Program at Grace United Church once a week 
 
People who are homeless and registered as clients at Bramalea CHC can access primary health care from a team that includes a nurse practitioner, doctor, registered nurse, dietitian, physiotherapist, social worker and community health educator 
 
Other services include: 
•Counselling to address a wide range of mental health and substance use issues 
•Support accessing other community based supports such as subsidized housing, shelters and food banks 
 
In addition to offering individual supports to individuals who are homeless, the agency also work on larger initiatives and partnerships to end social conditions that contribute to homelessness.

Eligibility Live in the area served * individuals or families living in shelters, or have no fixed address* women or men in domestic violence situation who may need to leave their home
