Description/Eligibility/Service Details
Description | Comprehensive diabetes education for people living with type 2 diabetes and those at risk of developing diabetes (pre-diabetes) The program and services are provided by certified diabetes nurse educators and certified diabetes registered dietitians * group or private appointments are available Educating people with type 2 diabetes to: reduce risk of complications deal with emotions connected to the disease manage blood sugar levels cope with stress control weight prevent and manage foot problems take medications correctly develop a healthy eating plan meet and learn from other people with diabetes Educates people at risk of developing diabetes to: reduce blood sugar levels lose weight label read /make healthy food choices create a realistic physical activity plan take medications correctly Contact the Diabetes Education Program for information. In person appointments are available if required. Currently individual appointments are in person, by phone or Zoom Diabetes Community Outreach: diabetes health education provided at community locations (religious, community organization, school, or work place), for groups of five or more adults living with Type 2 Diabetes or Pre-Diabetes |
