Description & Services

Description Supports for people living with prediabetes or diabetes * one-on-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * registered nurse and dietitian on staff 
 
Teaches people with type 2 diabetes to: 
reduce risk of complications 
deal with emotions connected to having the disease 
monitor sugar levels and keep within target range 
manage stress 
control weight 
prevent and manage foot problems 
take medications correctly 
develop a healthy eating plan 
meet and learn from other people with diabetes 
 
Teaches people at risk of diabetes to: 
bring down glucose levels 
design a plan to lose those few extra pounds 
learn how to read labels and make healthy food choices when grocery shopping 
get moving with a regular and realistic physical activity plan 
take medications correctly
Hours Mon, Wed 8am-8pm * Tue 1pm-4pm * Thu 8am-6pm * Fri 8am-4pm

Eligibility Live in the area served * be a member of one or more of the following priority populations: Youth (13-25) * adults 65 years and older * newcomers to Canada (less than 5 years) * racialized communities * people facing mental health conditions or substance use issues * people who are homeless or at-risk of homelessness * families who are under-resourced Residents of Bramalea with postal codes starting with L6T, L6R and L6S
