Description & Services
| Description | Supports for people living with prediabetes or diabetes * one-on-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * registered nurse and dietitian on staff Teaches people with type 2 diabetes to: reduce risk of complications deal with emotions connected to having the disease monitor sugar levels and keep within target range manage stress control weight prevent and manage foot problems take medications correctly develop a healthy eating plan meet and learn from other people with diabetes Teaches people at risk of diabetes to: bring down glucose levels design a plan to lose those few extra pounds learn how to read labels and make healthy food choices when grocery shopping get moving with a regular and realistic physical activity plan take medications correctly |
