Description & Services
Description The Diabetes Education Program offers comprehensive diabetes education to people living with:  
• Type 2 diabetes, or  
• At risk of developing diabetes (pre-diabetes)  
The program and its services are free of charge and delivered by diabetes nurse educators and registered dietitians. A group or private meetings are available.  
If you are living with Type 2 Diabetes, you will learn how to:  
• Reduce risk of complications  
• Deal with emotions connected to having the disease  
• Monitor sugar levels and keep within target range  
• Manage your stress  
• Control your weight  
• Prevent and manage foot problems  
• Take your medications correctly  
• Develop a healthy eating plan  
• Meet and learn from other people with diabetes  
If you are at risk of Diabetes, you will learn how to:  
• Bring down your glucose levels  
• Design a plan to lose those few extra pounds  
• Learn how to read labels and make healthy food choices when grocery shopping  
• Get moving with a regular and realistic physical activity plan  
• Take your medications correctly
