Description Rexdale Community Health Centre delivers integrated, coordinated clinical services through a team of physicians, nurse practitioners, nurses, dietitians, social workers, a physiotherapist and a chiropodist. Our multi-disciplinary team addresses the health needs of individuals and families at all ages and stages of their lives 
Rexdale Community Health Centre in partnership with other organizations offers: 
1. Free Dental Service in partnership with Toronto Public Health for children, high school students and seniors * service available to individuals living in the City of Toronto who are 65 years and older, or 0-17 years of age and attend school, do not have private dental insurance 
Service includes: examinations * prevention and education * fillings * fluoride treatment * root canal treatment * cleaning * extractions * sealants * partial and full dentures

2. Counselling / Support Programs in partnership with Family Services Toronto Access Unit (SAU) for individuals, couples and families living in the City of Toronto and experiencing life challenges. 
Services include: Counselling Service * The David Kelley Lesbian and Gay Community Counselling * The David Kelley HIV/AIDS Community Counselling * Families in Transition * Seniors and Caregivers Support * Next Steps (Partner Abuse Response) * Violence Against Women
