Description & Services

Description Program that refers people without a regular family health care provider to physicians and nurse practitioners who are accepting new patients in their area 
•voluntary program for both patients and participating family health care providers patients register by telephone 1-800-445-1822 or call 811, with a centrally located 
•patient representative, or, they can register online www.ontario.ca/healthcareconnect 
•patients must have a valid Ontario Health Card Number to be eligible for the program  
•once patients are registered, information is forwarded to Home and Community Care Support Services office that covers the area where the patient resides  
•Care Connectors are based locally in Home and Community Care Support Services offices and facilitate the connection between patients and doctors/nurse practitioners

Areas Served Brampton ; Caledon ; Dufferin County ; Etobicoke North ; Malton ; Woodbridge
Eligibility Patients must have a valid Ontario Health Card Number
