Description PCMH is Children's Mental Health Ontario's family partnerships program and is grounded in communities across Ontario, led by volunteer Peer Support Facilitators with lived experience in caring for and supporting children and youth with mental health challenges. They support, educate, and empower families through peer support, trusted resources, and ongoing webinars, as well as helping them navigate the mental health systems across Ontario.
