Program Description 
 
Kinark Child and Family Services 
Dual Diagnosis Systems Support Program 
 
The Kinark Peel Dual Diagnosis Systems Support Program will provide short term (3 to 6 months) enhanced service coordination and systems navigation to children/youth with an intellectual disability, neurodevelopmental disability and severe behavioural problems and/or mental health concerns and their families/caregivers and agencies in Peel Region while building capacity within the Children’s Mental Health and Developmental Services systems. The program will provide service through three primary components: 
 
1) Diagnostic Clarification: 
Our program will provide tertiary psychiatric assessment, consultation, and medication reviews, with consent by a physician in situations where additional opinion is required due to the complexity of the needs of the child/youth with an intellectual disability, neurodevelopmental disabilities and severe behavioural problems and/or mental health concerns. 
 
2) Systems Navigation/Service Coordination: 
 
Support children/youth with an intellectual disability, a neurodevelopmental disability and severe behavioural problems and/or mental health concerns and their families/caregivers to identify appropriate services available in their community 
 
Engage the professional expertise of various service system partners 
in a collaborative and integrated approach to service planning and delivery 
 
Facilitate problem-solving discussions with local service providers 
regarding access to services, and the delivery of services and supports 
 
Identify additional services and supports that may be helpful 
 
Seek specialized assessments or services as warranted 
 
Assist in the development and implementation of a discharge and/or transitional support plans 
 
Help to ensure service continuity and prevent duplication (e.g., ensure information is shared amongst service providers as much as possible within privacy constraints). 
 
3)Education and Knowledge Exchange: 
Provide educational support and facilitate exchange of knowledge in the Peel community in order to increase/sustain capacity of families/caregivers and agencies through, workshops, consultations and partnerships.

Resident of Ontario; custodial parent/s or guardian reside/s in the Region of Peel 
 
Up to the eighteenth birthday 
 
The child/youth has an intellectual disability, neurodevelopmental disability and severe behavioural problems and/or mental health concerns 
Dual Diagnosis Systems Support Program 
Program Description

A family or an agency is having difficulty gaining access to services due to the complexity or extraordinary needs of the child/youth with an intellectual disability, a neurodevelopmental disability, behavioural problems and/or mental health concerns 
 
Situation is sufficiently complex and/or atypical and there are difficulties identifying appropriate services available in their community 
 
The child/youth with an intellectual disability, neurodevelopmental and severe behavioural problems and/or mental health concerns is receiving services and is case managed by a community agency 
Eligibility Criteria for Psychiatric Services 
 
The child/youth has an intellectual disability, neurodevelopmental or mental health diagnosis Referral from Psychiatrist or family doctor 
 
For medication review referrals no other Psychiatrist is available to provide medication reviews 
 
The child/youth with an intellectual disability, neurodevelopmental disability or existing mental health diagnosis and has received a psychiatric assessment in the past 3 years 
 
Results of the assessment are inconclusive or incomplete 
or 
 
The Psychiatrist or family doctor of the child/youth with an intellectual disability, Fetal Alcohol Spectrum Disorder, or Autism Spectrum Disorder is looking for a second psychiatric opinion in complex cases 
 
A specialized assessment is being considered that is not covered by OHIP 
Definitions 
Complex/Extraordinary: 
 
Needs reflect multiple levels of intervention 
 
Multiplicity of need and complexity of the child/youth’s circumstances 
 
Discrepancy between the child/youth’s needs and the system’s ability to respond (e.g., availability of formal/informal supports in the family/community) 
 
Inability of agencies to respond based on mandates and services provided (i.e., type of service does not exist) 
 
Needs are beyond the resources, capacities and mandates of what is available in the service sector 
Referral Process: 
Service Coordinators or service providers acting as the main point of contact can refer children/youth through the use of a standardized intake package. Requests for diagnostic clarifications will include completion of consents for authorization of assessment from a physician.

Eligibility Ages: 18 year(s) and under 
 
Referrals can be made by Case Managers/Service Coordinators in collaboration with families. Referrals must be made before the youth's 18th birthday. Consultation can be provided for children under the age of 6.
