Description	Provides Education to enhance knowledge and skills of staff; consultation about learning needs, care plans and programs related to challenging behaviours of clients / residents. Assistance with referrals to services and resources and enhanced development of linkages between services providing dementia care.  Rresponsible for 9 Brampton area Long Term Care Homes : Burton Manor; Leisureworld Tullamore; Leisureworld Brampton Woods; Leisureworld Brampton Meadows; Specialty Care woodhall; Village of Sandalwood Park; Extendicare Brampton; Grace Manor and Faith Manor.
Hours	Mon- Fri. Hours depends upon staff / shift needs
Dates	According to mutually agreed to date and time. Benefical for as much advance notice as possible by requesting organization.
Areas Served	Brampton (City of) ; Caledon (Town of) ; Dufferin County ; Etobicoke North ; Malton ; Woodbridge
Boundaries	Long Term Care Homes (LTC) outside of Peel Region encouraged to call PRC to clarify contact information for specific PRC.
Eligibility	Must be a government funded agency such as: CCAC; Long Term Care Homes; Adult Day programs; Homemaking agencies; Supportive Housing; Respite Care programs dealing with dementia and mental health issues.
Application	Referrals can be made by anyone; within a government funded agency; who has identified a learning need related to the mental health of the elderly; or anyone concerned about an older adult who may have a psychological, behavioural or memory problem.
