Description	Service for people who are homeless/at risk for homelessness/under housed  People who are homeless and registered as clients at Bramalea CHC can access primary health care from a team that includes a nurse practitioner, doctor, registered nurse, dietitian, physiotherapist, social worker and community health educator  Other services include:   Primary care from nurse practitioner, physician and/or registered nurse at local shelters * Counselling to address a wide range of mental health and substance use issues * Support accessing other community based supports such as subsidized housing, shelters and food banks    To ensure easier access to the homeless/at risk of homelesssness/ under housed population, we provide health assessment and referrasl at the Regeneration Breakfast Program at Grace United Church once a week.
Hours	Mon, Wed, Thu 8 am-8 pm * Tue 1 pm-8 pm * Fri 8 am-4 pm * Sat 8 am-12 noon
Areas Served	Brampton ; Residents of Bramalea with postal codes starting with L6T, L6R and L6S.
Boundaries	Hwy 410 (east) , Hwy 407.(south), Airport Rd.(west), Mayfield (north)
Eligibility	Ages: 99 year(s) and under    To become a client of the Bramalea Community Health Centre you must 1) live in the area served and 2) be a member of one or more of the following priority populations;  - youth (13 - 25)  - seniors (65+)  - newcomers to Canada (less than 5 years)  - racialized communities  - people facing mental health conditions or substance use issues  - people who are homeless or at-risk of homelessness  - families who are under-resourced
