| Description | Rexdale C.H.C. is a family practice medical services centre in which family doctors and nurse practitioners work together in teams. Services Include: 1. Sexual Health Clinic Operates two Sexual Health Clinics in the community, in partnership with Toronto Public Health to provide access to information * education * treatment and medications that will help them live sexually healthy lives. Services include: Birth Control * Sexually Transmitted Infections, Pregnancy Testing and Counselling * Sexuality Questions * Hepatitis B Vaccine * HIV Testing and Counselling. Rexdale Community Health Centre - all ages 8 Taber Rd. Etobicoke 416-744-6312 Every Wednesday 3:00 - 7:00 pm Rexdale Youth Resource Centre - under 25 years only 1530 Albion Rd, Albion Centre, Etobicoke, 416-741-8714 Every Thursday 2:45 - 5:15 pm 2. Healthy Eating Groups Offers presentations that will encourage on-going physical activity and nutritional behaviour changes designed to support life-long fitness and good eating habits. Topics generally cover Canada's Food Guide to: Healthy Eating * Food Budgeting & Grocery shopping * Menu planning * Cooking skills and can be adapted to the diversity / multi-cultural needs of the community. 3. Chiropody (Foot Care) This service specializes in providing complete family foot care for diabetic children, seniors and adults. The services include: provision of orthotics & orthopaedic shoes * physical therapies * sport medicine * nail care and foot care * foot infections * foot deformities and misalingments 4. Diabetes Education Centre Registered Dietitians and Nurses offer educaion on how to manage and/or prevent type 2 diabetes. Individual counselling and group education provided on Healthy eating, Physical activity, self management, diabetes medication, foot care, stress management. No boundaries. 5. Nutrition Counselling & Services "Good Food Box" Community Dietician provides individual counselling on a range of nutritional issues: prenatal, infant feeding * weight management * diabetes * high cholesterol, and more. Offers group education to parents, staff and other community groups. The Good Food Box is a program designed for people who want to include fresh fruit and vegetables in their diet. The Good Food Box distribution system provides low-income people with improved access to nutritious foods in their community, while also encouraging community development, and support for the local agricultural economy. Provided twice a month on Tuesdays Cost $17-$32 6. Home Visits Family Physicians provide home visits for palliative care clients. Physicians will schedule regular home visits to ensure that the patient is receiving consistent care. 7. Flu Clinics Provides two free Flu Clinics every year during the month of November in partnership with Toronto Public Health. 8. Well Women Services Emphasis on women's health care - Female clients can book well women visits with a female health provider. Routine well women care includes: discussing your medical history * medications * breast health * self-breast examinations * annual pap smears * family planning * diet * exercise * alcohol and/or drug use * depression or anxiety. Well woman clinics are coordinated by Nurse practitioners. 9. Pre-and-Post Natal primary care for women and newborns Offers: prenatal and post natal care and obstetrical services for registered clients of the centre, by nurse practitioners and physicians, and clients are referred for delivery to an obstetrician. Perinatal Education: provides health education in the perinatal period through a variety of educational strategies and approaches. Core education targets healthy pregnancy * childbirth preparation and breastfeeding, and early parenting. Physiotherapy Offers a wide range of specialized servieces, from expert diagnosis to treatment and disease prevention, The practice of physiotherapy is the assessment of physical function and the steps taken to optimize physical ability. Offers: client instruction/education/advice on prevention, electrotherapeutic modalities, ultrasound, physical agents, manual therapy, general cardio, therapeutic exercise and more. | |||
