Description Medical care team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness * inter-professional team includes family doctors, nurses, nurse practitioners, registered dietitians, social workers, a chiropodist (foot care specialist), and a pharmacist * free seasonal flu vaccines * currently accepting new patients * also provides palliative care 
 
Diabetes Program - Diabetes care team, which may include a family physician, diabetes nurse educator, dietitian, optometrist, social worker, chiropodist, pharmacist and endocrinologist, helps patients manage their diabetes and maintain good health  
 
Smoking Cessation (STOP) Program - One-on-one support to help quit smoking * facilitate access to nicotine replacement therapy (NRT) * 6 months of nicotine patches, gums, inhalers or lozenges may be supplied 
 
Breath for Life - Provides lung function test (spirometry) * promotes health for patients with asthma of chronic obstructive pulmonary disease (COPD) * education on lung health along with medication management 
 
Cancer Screening - For early detection of cervical cancer (PAP test), colorectal cancer (FOBT test), and breast cancer (mammogram) 
 
Patients also have the option to have minor surgery done at the Family Health Team office instead of having it done at the hospital * patients are responsible to pay all costs associated with the procedure that are not covered by OHIP
