Description Program for seniors experiencing serious and persistent mental health concerns which include: depression, mood changes, dementia, responsive behaviours, and/or anxiety 
 
Seniors having challenges accessing supports due to geography, isolation or changes in informal support system are considered a priority for this service 
 
• makes referrals to, and collaborates with, community partners such as Local Health Integration Network (LHIN), Seniors Day Programs, Alzheimer’s Society of Dufferin and local hospitals 
• education and support to the client and care team, to ensure clients have the necessary services and supports to live successfully in the community and to maintain/improve quality of life
Hours Mon-Fri 9am-5pm

Application To complete a referral call 905-451-2123 For Rapid Access Addiction Clinic walk-in or call for appointment

Languages English ; Translation and interpretation is available for most languages. When you call please let us know if interpretation services are needed
