Description Program for seniors experiencing serious and persistent mental health concerns which include: depression, mood changes, dementia, responsive behaviours, and/or anxiety 
 
Seniors having challenges accessing supports due to geography, isolation or changes in informal support system are considered a priority for this service 
•makes referrals to, and collaborates with, community partners such as Home and Community Care Support Services Central West, Seniors Day Programs, Alzheimer’s Society of Dufferin and local hospitals 
•education and support to the client and care team, to ensure clients have the necessary services and supports to live successfully in the community and to maintain/improve quality of life
Hours Mon-Fri 9am-5pm

Eligibility Seniors with mental health issues
Application To complete a referral call 905-451-2123. For Rapid Access Addiction Medication, walk-in or call for appointment

Languages English ; Translation and interpretation is available for most languages. When you call please let us know if interpretation services are needed
