Description Collaboration with Service Providers in order to more adequately support persons with complex needs. Facilitates access and/or develop community plans for transitional aged youth and adults with complex mental health and /or addictions challenges to care that is coordinated, uninterrupted, developmentally appropriate, comprehensive and flexibly adapted to the needs of the client 
 
Represents coordination efforts for persons with complex needs from the following Child, Youth, and Adult Social Service sectors: Acquired Brain Injury, Addictions & Mental Health, Criminal Justice, Developmental Disabilities, and Dual Diagnosis
Hours Administration: Mon-Fri 9am-5pm
Areas Served Dufferin County ; Etobicoke North ; Peel Region ; Woodbridge
Eligibility Transitional Aged Youth Mental Health and Addictions (16-24) * Adult Mental Health and Addictions (18+)
