Description Group therapy for individuals with aphasia and other acquired communication disorders  
• emphasizes communication activities  
•focuses on effective communication, not just speech  
•provides support to family members  
•educates the community about aphasia

Eligibility - Varied types / severities of aphasia due to stroke or brain injury, e.g., Acquired Brain Injury (ABI), Primary Progressive Aphasia (PPA), tumor  
- Individuals must be medically stable and live in community  
- Willingness to participate in Supported Conversation Groups
Application Complete referral form and fax to (365) 601-1690 c/o Halton-Peel Community Aphasia Program for review and determination of eligibility to the program. Referrals can be made by Professionals in the community (Discharge Planners, Speech Language Pathologists, Social Workers, Physicians, etc.). Self-referrals (clients can refer themselves to the program), Family or Friends of the person with aphasia.
