| Description | - Nurse-Led Outreach Team-Nurse practitioners (NPs) at Osler partner with long-term care homes (LTCH) in the community to provide timely care to seniors and avoid unnecessary transfers to hospital. - Geriatric Outreach Team- a multidisciplinary team that specializes in caring for seniors living in the community The team provides in-home non-urgent geriatric assessment to frail seniors who are unable to attend clinic visits. The assessment focuses on the senior’s functional, psychosocial, cognitive and physical health status. The team serves patients living in the Central West LHIN - Senior Outpatient Clinic |
