Description | Intensive individual assistance and advocacy around accessing community resources and services to individuals experiencing mental health concerns, who are in conflict with the law •Court Diversion – connecting clients with community service providers to address their mental health concerns, instead of going through the court process •Release from Custody Planning – helping clients transition back into their home communities after being released from custody in Maplehurst Correctional Complex, Ontario Correctional Institute or Vanier Centre for Women - Fetal Alcohol Spectrum Disorder (FASD) Diagnostic Clinic -- Support with coordinating the different facets necessary for a diagnosis of FASD, FASD education and connection to resources. •Mental health support for individuals and their families, including access to group programs such as: peer support, fitness promotion, social recreation and family support •Connections to the broader community through volunteering, social or recreational activities, continuing education and support/connection to housing providers. |
