Description Access to adult outpatient services is streamlined through Centralized Intake and Triage (CIT). 
Referring psychiatrists or family physicians should complete a Centralized Intake and Triage Referral Form and fax it to either 
Etobicoke General: 416-747-3350  
Brampton Civic: 905-494-6715
Hours Mon-Fri 8:30am-4:30pm
Areas Served Brampton ; Caledon ; Dufferin County ; Etobicoke ; Malton ; Peel Region ; Other areas whose doctors have admitting privileges
Eligibility Ages: 18 year(s) and up
Application Referrals are welcome for patients and physicians within Osler’s catchment area, that is, the Central-West Local Health Integration Network (CW-LHIN). The CIT process Clinicians screen the patient and provide a brief assessment and referral. The referral may be for a consultation, consultation and treatment, or telephone advice, depending on the patient’s needs.
