Description Individual Grief and Bereavement Support 
Staff and qualified individuals provide individual grief and bereavement support to persons who have experienced or are anticipating the loss of a loved one 
Adult Grief/Bereavement Support Groups 
Throughout the year, eight week long support groups are offered to caregivers, families and/or friends following the death of a loved one. When possible, the groups are structured around a common loss, i.e., parents grieving the loss of a child 
Telephone Support 
Information and emotional/bereavement support is available in the form of telephone support from both hospice staff and/or trained volunteers. This is helpful to the elderly and disabled who may have difficulty with mobility
Hours Mon-Fri 10am-4pm
Areas Served Concord ; Kleinburg ; Woodbridge
Eligibility Persons with life threatening illnesses, caregivers or persons grieving death of a loved one
