Description Children attending school in Toronto must have an up-to-date immunization record or a valid exemption form on file with Toronto Public Health. 
Toronto Public Health provides immunization clinics in schools and community settings. All clinics are free, no health card is required. 
• Elementary school students need vaccinations or shots against diphtheria, tetanus, polio, and measles, mumps, and rubella;  
Beginning July 2014, the three new reportable vaccines are: meningcococcal 
whooping cough (pertussis) and chickenpox (born on or after 2010) 
• Students 14 to 16 years of age need a booster vaccine against diphtheria, tetanus and pertussis

(Send a clear photocopy only. Do not send the original yellow immunization card)  
Allow 72 hours for records to be updated in our database, when faxing information, or using the online system. 
School Clinics 
Each year Toronto Public Health provides school clinics for grade 7 and 8 students to receive hepatitis B, meningococcal and HPV vaccines. See school clinic dates for more information.

Community Clinics- 4 locations in Toronto area 
All community clinics are available by appointment only. You need to register first before booking an appointment. There are four community clinics offered by Toronto Public Health. To be eligible, students must be attending a Toronto school and: 
•be between the ages of 4 and 17 years 
•have a sign consent form from their parent/legal guardian 
•high school students, age 14 and up can sign their own consent form
