Description Telehomecare program links patients from home to registered nurses at Brampton Civic. Using telephone and internet communication, as well as vital signs monitoring devices, nurses monitor and support people to manage conditions such as heart failure or chronic obstructive pulmonary disease and chronic heart failure. 
 
This program is funded by the Ministry of Health and provided in partnership with the Ontario Telemedicine Network.
Hours Mon-Fri 8:30am - 4:30pm
Areas Served Dufferin County ; Etobicoke North ; Peel Region ; Woodbridge
Eligibility Patients with an established diagnosis of a congestive heart failure (CHF) or chronic obstructive pulmonary disease (COPD) 
Repeated emergency department visits and/or hospital admissions 
Capable of using in-home monitoring equipment
Application Health care providers can enrol patients in the program by faxing a completed referral form to 905-494-6784 or contacting the program by phone: 1-855-683-7112
