Description Nursing clinics operated by Local Health Integration Networks (LHINs) provide nursing services to current LHIN patients including wound care, medication management and disease management  
Home and community care services and staff transferred to Local Health Integration Networks. Home and community care services are now provided through Ontario’s 14 Local Health Integration Networks
Hours Daily 8am-9pm (includes holidays) * Closed for lunch 1pm-2pm * Last appointment is between 8pm-8:30pm
Areas Served Brampton ; Caledon ; Malton
Eligibility CW LHIN patients * able to travel to Nursing Care Centre using public or private transportation * ambulate independently / minimal aids * requires BID visits or less * treatment can be managed within nursing care centre hours * service can be provided safely in the Nursing Care Centre setting * condition is stable, and patient is able to attend Nursing Care Centre setting without undue hardship or risk
Application Patients are assessed for service by Central West LHIN staff
