Description & Services

Description Service for older adults and adults with health challenges like depression, anxiety, or other mental health concerns.  
 
Service include: 
 
•an intake assessment-in client home or other convenient location 
•short or long term case management-building a supportive partnership, providing assistance in managing illnesses, and linking with other community resources  
 
A partnership initiative between CANES Community Care, Canadian Mental Health Association, Peel Dufferin Branch and Reconnect Community Health Services
Hours Office: Mon-Fri 8:30am-4:30pm
Areas Served Brampton ; Etobicoke North ; Mississauga - Malton ; Woodbridge
Eligibility Seniors living in the Etobicoke, Woodbridge, Malton and Brampton areas (Home and Community Care Support Services Central West) Seniors (65 yrs and up) with a mental health issue (dementia excluded). Adults over 55 yrs can also be seen if they have complex medical issues in addition to the mental health issue
Application Referrals can be made by an individual, a family member, or a professional; by telephone, fax or sent electronically by downloading the referral form from CANES website
